Authorization to verify. I authorize Rising, LLC and its agents to verify the information in this application directly with employers, financial institutions, government benefits providers, prior landlords, and any other party reasonably necessary to confirm income, assets, and household composition.

Release. I authorize each source named in my application to release the requested information to Rising, LLC for the purpose of determining income eligibility for a MAHT-restricted rental unit. This release is valid for 90 days from the date signed and may be revoked in writing at any time. A photocopy of this release shall be treated as the original.

Certification. I certify under penalty of perjury that the information provided in this application is true and complete to the best of my knowledge. I understand that intentional misstatement may result in denial of tenancy or termination of an executed lease, and is grounds for legal remedies available to Rising, LLC and the Maryland Affordable Housing Trust.
